Licensed Practical Nurse

Qualifications

• Current, unrestricted license to practice as a Licensed Practical Nurse (LPN) in the State of Georgia
• Minimum of two (2) years of experience in home care, long-term care, or a related healthcare setting preferred
• Experience working with elderly individuals, persons with physical or mental disabilities, and veterans preferred

Knowledge of:

• Home care service delivery
• Personal care services within LPN scope of practice
• Observation and reporting of client condition changes
• Accurate clinical documentation
• Ability to meet Georgia DCH background screening requirements
• Current TB clearance per agency policy
• Current CPR and First Aid certification

Education:

Graduation from an accredited practical nursing program with appropriate licensure to practice as an LPN

Duties & Responsibilities:

• Perform nursing functions within the scope of practice under the direction of the Supervising RN and/or Administrator
• Participate in client visits as assigned to observe client condition and service effectiveness
• Assist with the implementation and updates of client service plans under RN direction
• Complete nursing notes, visit documentation, and required clinical records accurately and timely
• Observe PCA/CNA service delivery and report concerns or changes in client status to the Supervising RN or Administrator
• Assist with staff training and competency support as assigned
• Communicate client progress, concerns, or incidents promptly to the Supervising RN or Administrator
• Comply with Georgia DCH regulations, agency policies, and documentation standards
• Perform other duties as assigned by the Administrator within LPN scope of practice

Job Type: Part-time

Pay: Up to $30.00 per hour

Expected hours: No more than 20 per week
