Job Overview
We are seeking a dedicated and experienced Primary Care Provider to deliver comprehensive healthcare services within a clinical setting. The ideal candidate will possess strong clinical skills, familiarity with electronic health record (EHR) systems, and a commitment to patient-centered care. This role involves managing a diverse patient population, performing thorough assessments, and coordinating care across various specialties. The Primary Care Provider will play a vital role in promoting health, preventing disease, and managing acute and chronic conditions.

Duties

• Conduct comprehensive medical history reviews and physical examinations, including vital signs and medical terminology assessment.
• Diagnose and treat a wide range of acute and chronic illnesses utilizing knowledge in areas such as family medicine, internal medicine, pediatrics, geriatrics, cardiology, endocrinology, pulmonology, neurology, and more.
• Develop individualized care plans based on clinical evaluation and patient needs, incorporating disease management protocols such as diabetes care, hypertension management, immunizations, wound care, and post-operative care.
• Utilize advanced diagnostic tools including medical imaging (CT scans), diagnostic radiology, nuclear medicine, and laboratory results to inform clinical decisions.
• Manage medication administration and perform procedures such as suturing, wound care, ostomy care, IV infusion, and airway management following aseptic techniques.
• Coordinate with specialists for referrals involving cardiology (including cardiac catheterization), nephrology, oncology, reproductive endocrinology, infectious disease care, neurocritical care, pediatric critical & intensive care, trauma medicine (Level I & II trauma center experience), and other subspecialties.
• Document clinical encounters accurately using EMR/EHR systems like Epic while ensuring compliance with HIPAA regulations.
• Participate in clinical research or trials when applicable to enhance evidence-based practice.
• Engage in patient education and health coaching to promote wellness initiatives such as family planning, nutrition counseling, smoking cessation programs, mental health support including PTSD care or behavioral health services.
• Assist in discharge planning for hospitalized patients or those transitioning to post-acute or home care settings.

Experience
Candidates should demonstrate extensive experience in outpatient primary care settings with proficiency in managing diverse patient populations across age groups. Experience with inpatient hospital medicine or ICU/PCU environments is highly desirable. Familiarity with advanced procedures such as catheterization (vascular interventional radiography), spinal tap, gastric lavage, or sleep medicine is advantageous. Prior work involving telehealth platforms is preferred. Candidates must be comfortable working with complex cases involving multiple comorbidities like rheumatology conditions or immunology disorders. Additional qualifications include proficiency with medical coding (CPT/ICD), familiarity with interventional radiology techniques when applicable, knowledge of diagnostic evaluation tools like PET scans or mammography procedures, and experience adhering to NCQA standards for quality healthcare delivery.
