Job Summary
We are seeking a compassionate and highly skilled Hospice Case Manager to join our healthcare team. The ideal candidate will coordinate patient care, facilitate communication among multidisciplinary teams, and ensure that patients receive comprehensive, compassionate end-of-life care. This role requires a strong background in various clinical settings, including acute, post-acute, outpatient, and inpatient care, with a focus on hospice and palliative services. The Hospice Case Manager will play a vital role in discharge planning, medical records management, and patient assessment to promote quality outcomes and patient comfort.

Duties

  • Conduct thorough patient assessments to determine care needs and develop individualized care plans aligned with hospice protocols.
  • Coordinate with physicians, nurses, social workers, and other healthcare professionals to deliver holistic patient care.
  • Manage discharge planning processes to ensure smooth transitions from hospital or facility to home or hospice setting.
  • Oversee utilization management and diagnostic evaluations to optimize resource use while maintaining high-quality care standards.
  • Maintain accurate medical records in compliance with HIPAA regulations and hospital policies.
  • Monitor vital signs and perform routine patient service activities including symptom management and medication reconciliation.
  • Facilitate intake procedures for new hospice patients, ensuring timely and appropriate placement within the care continuum.
  • Provide education to patients and families about disease progression, treatment options, and comfort measures.
  • Ensure adherence to infection control protocols and safety standards within all clinical activities.
  • Collaborate on post-acute care planning for patients transitioning from inpatient or outpatient settings to hospice or palliative care.

Experience
Candidates should possess extensive clinical experience across various healthcare environments. Experience in managed care, discharge planning, utilization management, diagnostic evaluation, and case management is highly desirable. Familiarity with medical terminology, physiology knowledge, HIPAA compliance, medical records documentation, and patient assessment techniques is essential. Prior experience in inpatient or outpatient hospice or post-acute care settings is preferred.
