Responsibilities

• Provide physical therapy based on a physician's written care plan
• Evaluate patients' mobility, strength, range of motion, and home environment
• Design and implement personalized therapy programs
• Educate patients and families on proper use of equipment and exercises
• Report patient progress or changes to physicians
• Participate in discharge planning and interdisciplinary case conferences
• Supervise Physical Therapy Assistants (PTAs) per state and agency policies
• Document all visits, progress, and care plans accurately and promptly
• Refer patients for additional home health services when needed

Qualifications

• Degree in Physical Therapy from an accredited program (Bachelor's or Master's)
• Active PT license in California
• Minimum of 2 years of experience as a Physical Therapist (home health preferred)
• Current CPR certification
• Valid driver's license and insured vehicle in good working condition
• Strong communication and organizational skills
