Case Manager Job Description (QMHA) Overview: The Case Manager provides care coordination, case management, and supportive services to youth ages 6–24 and their families who are receiving outpatient mental health services at Transform Youth and Family Counseling. This role focuses on reducing barriers to care, supporting stabilization, and assisting families in navigating systems and community resources—particularly for higher-acuity youth.
The Case Manager functions as part of a team-based care model and works closely with therapists, caregivers, and clinical leadership.
This is a non-exempt, hourly position.

Duties / ResponsibilitiesCase Management & Care Coordination
- Provide case management services consistent with QMHA scope of practice to youth and families receiving outpatient mental health treatment.
- Connect youth and families with appropriate community resources and support systems to address behavioral health, educational, social, and daily living needs.
- Deliver at least 22 billable hours per week.
- Support prior authorization processes for psychiatric services by coordinating documentation, submissions, and follow-up with insurance payers under the direction of licensed prescribing clinicians
- Support families in navigating systems related to education, healthcare, and social services.
- Coordinate care with therapists, caregivers, schools, medical providers, and community agencies.
- Participate in high-acuity staffing meetings and team-based care coordination as assigned.
- Support safety planning and follow-up in collaboration with the clinical treatment team.
- Deliver services under the direction and supervision of licensed clinical staff, as required by OAR.
Collaboration & Communication
- Work collaboratively with therapists, the Caregiver Support Track (CST) team, and clinical leadership.
- Communicate relevant updates, barriers, and care coordination needs to the treatment team in a timely manner.
- Provide timely support to therapists and the treatment team by assisting clients and caregivers in connecting with appropriate community resources and service systems.
- Maintain professional, respectful, and collaborative relationships with internal and external partners.
Documentation & Compliance
- Complete timely and accurate documentation in the Electronic Health Record, including case management notes, contact notes, and required care coordination documentation.
- Ensure documentation meets agency standards and applicable Oregon Administrative Rules (including OAR , Medicaid requirements, and Transform Youth and Family Counseling policies.
- Maintain client confidentiality in accordance with HIPAA and agency policies.
Administrative & Professional Responsibilities
- Maintain regular, reliable attendance and timely arrival to work.
- Attend required staff meetings, trainings, and supervision.
- Participate in at least one community engagement activity per year.
- Conduct all duties in a professional manner that supports the reputation and mission of Transform Youth and Family Counseling.
- Other duties may be assigned as needed.
Qualifications and Skills
- Must meet, or be eligible to meet, the qualifications of a Qualified Mental Health Associate (QMHA) as defined by Oregon Administrative Rules (OAR
- This includes:
- A bachelor's degree in a behavioral science field or
- A combination of education and relevant experience that meets QMHA criteria under Oregon Administrative Rules.
- Ability to function within the scope of practice for a QMHA and under appropriate clinical supervision.
- Experience working with youth and families, particularly those with higher-acuity needs, strongly preferred.
- Knowledge of community resources and service systems relevant to youth and families.
- Strong organizational, communication, and time-management skills.
- Ability to work collaboratively as part of a multidisciplinary treatment team.
- Commitment to working in an LGBTQIA+ inclusive, social justice-oriented practice.
- Must have a professional, distraction-free workspace if providing any remote services.
- Must have reliable transportation as the candidate must be able to travel between all office locations in Southern Oregon.
Pay
Starting at $25/hour DOE. Pay differential offered for bilingual candidates.
Pay: From $25.00 per hour
Expected hours: 32.0 – 40.0 per week
