Kearny Wellness is seeking a highly skilled, compassionate, and patient-centered Nurse Practitioner to join our growing multidisciplinary care team. This role is ideal for a provider who values holistic, preventive, and evidence-based care while managing both acute and chronic conditions. The Nurse Practitioner will play a key role in delivering high-quality care to our diverse patient population across the lifespan, with services provided in outpatient, community-based, and telehealth settings.
The ideal candidate brings strong clinical judgment, excellent communication skills, and experience working collaboratively within a wellness-focused practice that emphasizes continuity of care, patient education, and long-term health outcomes. Proficiency with modern EMR/EHR systems and strong documentation skills are essential.
Duties & Responsibilities
- Conduct comprehensive physical examinations and health assessments for pediatric, adult, and geriatric patients at Kearny Wellness.
- Diagnose, treat, and manage acute conditions, chronic diseases, and complex medical needs using evidence-based clinical practice.
- Develop and implement individualized treatment and wellness plans, including preventive care, disease management, and lifestyle interventions.
- Perform in-office procedures as appropriate, including injections, venipuncture, IV therapy, catheterization, wound care, and minor procedures.
- Prescribe and administer medications safely in compliance with state regulations, HIPAA standards, and Kearny Wellness protocols.
- Accurately document patient encounters using EMR/EHR systems
- Order, interpret, and coordinate diagnostic testing such as laboratory studies and medical imaging.
- Provide care coordination, referrals, and follow-up to ensure continuity across specialists, hospitals, and post-acute services.
- Deliver patient education and health coaching focused on nutrition, chronic disease prevention, medication adherence, and overall wellness.
- Participate in telehealth visits to improve access to care while maintaining quality and compliance standards.
- Collaborate closely with physicians, behavioral health providers, care coordinators, and support staff within the Kearny Wellness team.
- Support population health initiatives, post-acute care planning, and care transitions as needed.
Requirements & Qualifications
- Active Nurse Practitioner license with current national certification and authorization to practice independently in the state of employment.
- Clinical experience in primary care, family medicine, internal medicine, urgent care, or community health preferred.
- Experience caring for diverse and vulnerable populations, including pediatrics, geriatrics, and patients with chronic or complex conditions.
- Proficiency with EMR/EHR platforms, if not comfortable with self-training
- Strong knowledge of anatomy, physiology, diagnostics, and pharmacology.
- Familiarity with CPT and ICD-9/ICD-10 coding for accurate documentation and billing.
- Demonstrated ability to perform common clinical procedures and manage infection control standards.
- Excellent interpersonal, communication, and patient education skills.
- Ability to work independently while thriving in a collaborative, team-based wellness environment.

Pay: From $65.00 per hour
