*Reports To:* Physical Therapy Director
*Qualifications:* Bachelor’s Degree in Physical Therapy with valid current California providers license.
*Responsibilities:* Provide high-quality, comprehensive orthopedic care by developing and implementing rehabilitative programs. Maintain a daily patient schedule and keep up to date records in the patient’s electronic chart. Support the Physicians and Physician Assistants as needed and communicate any concerns regarding the patient’s progress or care.
*Physical Requirements:*
Capable of assisting patients on and off exam tables, x-ray tables, and in and out of wheelchairs. Bending, reaching, stooping, frequently in an eight hour shift. Ability to demonstrate exercises, stretches and physical activities given for patient care.
*Duties:*
* Assess, plan, organize, and carry out rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering from disease or injury.
* Test and measure patient's strength, flexibility, motor development and function.
* Identify and document goals, anticipated progress and plans for reevaluation.
* Provide educational information about physical therapy plan, expected benefits, reasonable alternatives and injury prevention.
* Administer manual exercises and treatment to help relieve pain, increase the patient's strength, and decrease or prevent deformity.
* Develop and instruct patients and family about home exercise improvement programs.
* Record prognosis, treatment, response, and progress in the patient's electronic medical record.
* Discharge patient from physical therapy when goals or projected outcomes have been reached.
* Provide patients with appropriate follow-up care or referrals as needed.
* Develop and maintain positive working relationships with practitioners in order to properly plan, implement and assess therapy programs
* Evaluate, fit, and adjust recommend durable medical equipment.
