Job Summary

We are seeking a dedicated Occupational Therapist to join our team. This role plays a key part in helping patients achieve their rehabilitation goals and regain independence in their daily lives. The Occupational Therapist will work closely with patients and their families to develop and implement individualized care plans that support functional ability, safety, and overall quality of life within the home setting.

Duties/Responsibilities:

  • Provide occupational therapy services in accordance with the physician's plan of care.
  • Evaluate and assess patient functional abilities and assist in developing and updating individualized care plans.
  • Deliver therapeutic interventions to improve function, self-care, and independence, seeing multiple patients per day.
  • Monitor patient response to treatment and communicate changes to the physician and care team.
  • Educate patients, caregivers, and staff on therapy goals, techniques, and use of adaptive equipment.
  • Reassess patient needs regularly and revise plans of care as appropriate.
  • Maintain accurate, timely clinical documentation in compliance with agency policies.
  • Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
  • Participate in quality improvement initiatives, staff meetings, and ongoing education.
  • Provide required supervision of the Occupational Therapy Assistant (OTA).

Physical Requirements

  • Ability to sit, stand, and drive for extended periods while providing in-home care.
  • Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
  • Regular use of a tablet for documentation, communication, and training.

Qualifications
  • Graduate of an accredited Occupational Therapy program (ACOTE or equivalent) (required).
  • Successfully completed the NBCOT certification exam (or meets Medicare Conditions of Participation requirements if licensed prior to 1978).
  • Licensed as an Occupational Therapist in the state which you are applying (required).
  • Minimum of (1) year experience as an Occupational Therapist (required)
  • Valid driver's license, proof of auto insurance, and reliable personal transportation (required)
