The Health at Home Chaplain provides compassionate spiritual, emotional, and psychosocial support to hospice patients, families, and caregivers in accordance with the hospice philosophy of care. The Chaplain serves as a core member of the interdisciplinary hospice team, addressing spiritual distress, supporting end-of-life coping, and honoring the diverse spiritual, religious, cultural, and existential needs of individuals and families. Duties may include but are not limited to:
·  Conduct comprehensive spiritual assessments for hospice patients and families.
·  Develop, implement, and document individualized spiritual care plans consistent with patient goals and beliefs.
·  Provide spiritual counseling, prayer, rituals, sacraments, or non-religious support as requested.
·  Support patients and families through anticipatory grief, dying, death, and bereavement.
·  Respect and affirm diverse faith traditions, belief systems, and cultural practices.
Interdisciplinary Team Collaboration
Participate actively in interdisciplinary team (IDT) meetings, care planning, and case conferences.
Communicate spiritual care needs and observations to the hospice team.
Collaborate with nursing, social work, medical providers, and volunteers to ensure holistic care.
Family & Caregiver Support
Provide emotional and spiritual support to families and caregivers.
Assist families with meaning-making, life review, forgiveness, reconciliation, and legacy work.
Offer crisis intervention and spiritual presence during times of decline or death.
Bereavement & Community Support
Support hospice bereavement services as needed, including follow-up with families.
Coordinate or refer to community clergy or spiritual leaders when appropriate.
Participate in memorial services or hospice-sponsored remembrance events.
Documentation & Compliance
Document spiritual assessments, interventions, and care plans in the electronic medical record in compliance with hospice regulations.
Adhere to all hospice policies, procedures, ethical standards, and confidentiality requirements (HIPAA).
Maintain professional boundaries and ethical conduct at all times.
Education & Program Support
Provide education to staff and volunteers on spiritual care, grief, and cultural sensitivity.
Support hospice volunteers and staff experiencing grief or compassion fatigue.
Participate in quality improvement and performance initiatives as requested.

JOB DESCRIPTION:
Provide social work services to patients in acute care, maternal child care, emergency department, home health/ hospice setting for patients with acute, chronic, or terminal illness and their families. Services to patient and families include: identification of psychosocial needs, develops, and implements plan of care from the strengths perspective responsive to patient/family concerns, health care status, available resources, age specific problems and clinical impressions; discharge planning, crisis intervention, information and referral, protective services, patient family education, collaboration with the multidisciplinary care team, and building connections with community resources.
QualificationsJOB QUALIFICATIONS:
Experience:
Minimum Required Experience: 1-year equivalent work experience.
Preferred Experience: 3+ years equivalent work experience.
Education:
Minimum Required Education: Bachelor's degree required; Master of Divinity (MDiv) or equivalent theological degree preferred.
