Duties

• Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to promote independence and dignity
• Monitor patients' health by taking vital signs, observing for changes in condition, and reporting findings to healthcare professionals promptly
• Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance
• Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness and food safety standards
• Operate assistive devices such as Hoyer lifts or wheelchairs to facilitate safe transfers and mobility assistance
• Maintain a clean, safe environment by performing light housekeeping duties including cleaning patient areas, laundry, and sanitizing equipment
• Document care activities accurately in compliance with healthcare regulations and organizational policies

Skills

• Proven experience in home & community care or long-term care environments with knowledge of assisted living, nursing home, or hospice care practices
• Strong understanding of patient observation techniques including dementia care, Alzheimer's care, memory care, autism support, and working with individuals with developmental disabilities or disabilities
• Certification in CPR and First Aid to respond effectively during emergencies
• Familiarity with HIPAA regulations to ensure patient confidentiality at all times
• Ability to work with diverse populations including seniors, individuals with disabilities, or those requiring specialized memory or behavioral support
• Excellent communication skills for collaborating with healthcare teams, residents, families, and other caregivers
