JOB DUTIES / RESPONSIBILITIES

ESSENTIAL DUTIES

  • Engages the parent/caregiver in the process of obtaining informed consent at admission.
  • Orients the child and family to building, unit, programs and policies/procedures.
  • Completes a psycho-social assessment with the child and family upon admission that includes the development of an individualized comprehensive plan of care plan.
  • Assesses the need for assistance with benefits, entitlements, and/or other supportive services and provides assistance or referrals as needed.
  • Initiates and facilitates review and completion of Level II PASSR screening for appropriate residents in anticipation of day 365 of admission.
  • Collaborates with the family, starting at the time of admission, to assess any potential barriers to discharge and develop individual plan of care.
  • Promotes the concept of patient and family-centered care by encouraging family involvement and participation in resident's overall treatment plan including but not limited to - family meetings, education and training, entitlements, available onsite supportive services and any other aspects of care.
  • Serves as a model for staff in areas related to Patient and Family Centered Care by demonstrating an awareness and competency in the core concepts/principles.
  • Coordinates and facilitates family meetings.
  • Participates in weekly comprehensive care planning team meetings and unit rounds.
  • Responsible for assuring that all documentation is completed and prepared in a timely fashion while meeting all departmental, organizational and regulatory guidelines and standards.
  • Provides supportive services to residents as needed to facilitate their adjustment to the hospitalization, illness, or to address any ongoing concerns.
  • Provides supportive services and concrete interventions to families that foster and strengthen the child/family relationship.
  • Makes arrangements for children to have therapeutic home visits when applicable.
  • Acts as an advocate to the resident and family as needed.
  • Serves as a liaison with existing agencies and initiates new referrals to outside community, educational, social service, and behavioral health agencies.
  • Initiates mandated reports to the NYC Abuse and Maltreatment Registry when indicated and provides on-going communication with ACS/CPS agencies for all open and current cases of abuse/neglect/mistreatment.
  • Provides hospital on-call social work coverage as per schedule.
  • Ensures availability of social work services 7-days and in evenings through adherence to pre-determined work schedule and/or collaboration for coverage.
  • Works collaboratively with Social Work Supervisor in clinical supervision and scheduling while adhering to best practice and meeting regulatory standards and expectations.
