Summary: HCC Intensive Care Manager will provide case management, planning, and resource coordination for persons experiencing behavioral and mental health barriers as a factor in homelessness. This position requires knowledge and experience in housing first, trauma informed care, harm reduction, motivational interviewing, benefits eligibility, and assessing, diagnosing, and treating mental health and substance use disorders. Responsible for partnering with Guests to develop an Individual Service Plan for mental, behavioral, and physical care that leads to stability and recovery; maintaining accurate records; timely and accurate documentation; and providing appropriate referrals and linkages to supportive services that lead to sustainable homeless recovery. In addition, this position is responsible for providing housing follow up services to Guests who have been placed in housing. Housing follow up duties include face to face assessments at the Guest's home, linkage to community supportive services, and responding to needed telephone and email interventions.

Essential Functions:

· Partner with the Guest to develop an Individual Service Plan outlining specific, measurable, attainable, realistic, and time-limited goals and objectives that support recovery from homelessness.

· Identify and assess mental health and substance use disorders, engage in treatment, and provide appropriate referrals as needed.

· Monitor the level of care according to a Guest's needs and influence the progression of care; facilitating the Guest's navigation and proposing an appropriate transition plan.

· Enroll and provide care coordination for high need Guests, including but not limited to Guests with severe mental, behavioral, and medical health diagnoses who have difficulty independently managing their own care.

· Maintain timely and accurate documentation of all engagements, services, and referrals provided.

· Work with partners and other providers to ensure that the Guest's medical and behavioral health needs are managed.

· Assist Guests with locating providers, scheduling and transporting to appointments as needed.

· Research and refer Guests to community resources, such as health care and other agencies and community resources.

· Promote adherence to clinical protocols through collaboration with the team.

· Identify complex psychosocial and continuity of care needs that may present obstacles to the progression of care and make referrals for other services as needed.

· Serve as an advocate for the guest throughout their entire stay at the Bridge and three months following housing placement.

· Conduct post-placement in-home visits to support Guests for three months after placement.

Knowledge Skills and Abilities:

· Ability to quickly establish rapport, trust, and maintain appropriate boundaries with Guests.

· Capable of working independently to achieve stated program goals with minimum day to day supervision.

· Knowledge of psychosocial theory, best practice methods in care management, and social work ethics.

· Knowledge and experience in housing first, trauma informed care, harm reduction, motivational interviewing, benefits eligibility, and assessing and treating mental health and substance use disorders.

· Experience with homeless management data tracking systems such as Client Track a plus.

· Knowledge of local community resources.

· Ability to motivate and lead others in an ethical and responsible way towards organizational goal achievement.

Minimum Qualifications:

· · Bachelor's degree in social work or related field. Masters preferred.

· Three to five years' experience in social work or human services field to persons who are experiencing homelessness and have mental health and/or substance use barriers.

· LBSW, LMSW, or equivalent licensure in related field. Must be licensed in Texas.

· Clinical Skills: Must possess knowledge of and be able to effectively utilize evidence-based models of treatment and interventions, including but not limited to: motivational interviewing, active listening, crisis intervention, and trauma informed care. Must understand and adhere to the social work code of ethics.

· Proficiency in Word, Excel, and Outlook.

· Reliable transportation (automobile), current driver's license and valid liability insurance.

· Bilingual Preferred.

Important Qualities:

· Compassion : Must have compassion for homeless population and be able to show compassion in stressful and difficult situations

· Interpersonal Skills : Must be able to listen actively, establish rapport, and maintain appropriate boundaries while working with diverse groups of people. Foster healthy and productive relationships with Guests, partners, and staff.

· Organizational & Time Management Skills : This position requires that all reporting is accurate, well documented, and completed in a timely manner. Strong organizational and time management skills are necessary to effectively balance necessary documentation and daily needs of Guests.

· Problem Solving Skills: This position works with Guests, housing navigators, care management team and leadership to solve any issues that prevent housing placement or may threaten continuation of housing for program participants.

Working Environment:

This position carries the possibility of exposure to inside environmental conditions such as infectious/communicable diseases, blood and bloodborne diseases, odors, and dust. This position requires travel within the community for housing follow up sessions with housed Guests as well as transportation of Guests to appointments and resources outside of the Bridge Campus.

Job Type: Full-time

Pay: Up to $53,000.00 per year
