Licensed Social Worker

Overview
Licensed Clinical Social Worker (LCSW) to join our team. TruCare Living Center - Palestine is a 5 Star skilled nursing facility with established leadership. We serve our community by providing both short term rehabilitation and long term care.

Duties

• Conduct BIMS and PHQ-9 assessments
• Collaborate with multidisciplinary teams to coordinate care and ensure comprehensive care plans for clients.
• Engage in mentoring and educational activities to empower clients and promote their personal growth.
• Maintain accurate documentation of client progress
• Discharge planning from the day of admission

Skills

• Proficiency in crisis management techniques and crisis intervention strategies.
• Strong background in behavioral health practices and social work methodologies.
• Knowledge of addiction counseling techniques and motivational interviewing skills to facilitate client engagement.
• Familiarity with Medicare processes and regulations related to mental health services.
• Excellent communication skills, both verbal and written, for effective interaction with clients, families, and other professionals.
• Ability to work collaboratively within a team environment while maintaining a client-centered approach.
