What Are We Looking For?

Your Health is currently looking for a Pharmacist to join our growing primary care family. The Pharmacist is a specialized healthcare professional who will work in our Pharmacy here at SC Home RX. The Pharmacist will provide patient consultation, recommendations, and fulfill medication needs. A successful Pharmacist will be able to perform these essential duties and responsibilities. Review records of patients to determine the appropriateness of medication and fulfill medication as needed. Develop an effective medication recommendation that minimizes the risk of adverse side effects and ensure appropriate drug therapy. Collaborate with healthcare professionals to ensure optimal patient care. Accurately document medication plans and patient progress in the medical record, as well as ensuring proper billing of visits. Interact with patients via phone, email, patient portal, telephone, text, in person, etc. to gather necessary information.
