Job Summary
We are seeking a compassionate and dedicated Day Program Support Specialist to assist individuals with developmental disabilities and other needs in a community-based setting. The ideal candidate will provide personalized support, promote independence, and ensure safety while fostering a positive environment for participants. This role involves working closely with clients to assist with daily activities, health monitoring, and community engagement, contributing to their overall well-being and quality of life.

Duties

  • Assist participants with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor and document vital signs and health status in accordance with care plans
  • Administer medications and assist with medication management following HIPAA guidelines
  • Support participants during meal preparation, cooking, and nutritional planning
  • Provide personal care such as hygiene assistance and light cleaning of living areas
  • Utilize specialized equipment like Hoyer lifts for safe transfers and mobility assistance
  • Observe and report changes in behavior or health status through patient observation techniques
  • Implement individualized care plans tailored to each participant's needs, including dementia, Alzheimer's, autism, or other disabilities
  • Engage clients in social activities and community outings to promote socialization and independence
  • Maintain a safe environment by adhering to safety protocols including CPR and first aid procedures
  • Ensure compliance with all healthcare regulations, including HIPAA standards for confidentiality

Experience

  • Prior experience working in assisted living, home & community care, or long-term care settings preferred
  • Experience working with individuals with developmental disabilities or special needs such as autism or dementia is highly desirable
  • Knowledge of patient monitoring, medication administration, and vital signs measurement is advantageous
  • Previous roles involving patient care, nursing home experience, hospice or home health care are beneficial
  • Familiarity with care plans, resident support strategies, and social work principles is a plus
  • Certifications such as CPR and First Aid are preferred but not required; training will be provided if necessary
  • Ability to perform heavy lifting when necessary for client transfers or mobility assistance
