MDS Coordinator (RN/LPN)

We are seeking an experienced MDS Coordinator to join our skilled nursing team. The right candidate will be detail-oriented, organized, and knowledgeable in MDS assessments, care planning, and regulatory compliance.

Responsibilities include:

• Completing and submitting MDS assessments accurately and on time
• Assisting with care plans and interdisciplinary team meetings
• Ensuring compliance with federal and state regulations
• Supporting quality assurance and clinical reimbursement initiatives

Qualifications:

• Current RN or LPN license (state of practice)
• Previous MDS experience required
• Strong knowledge of RAI process, PDPM, and reimbursement guidelines
• Excellent communication and organizational skills
