Social Services Supervisor
The LIFE PACE (Program of All-Inclusive Care for the Elderly) team performs a vital service by providing comprehensive, community-based care for Tulsa's aging population. Our mission is to help participants remain safely in their homes for as long as possible through coordinated medical, social, and supportive services—offering a meaningful alternative to nursing facility placement. We are seeking an experienced and compassionate Social Services Supervisor to join our PACE Leadership Team. This role provides leadership and oversight for the delivery of comprehensive social work services to PACE participants, ensuring high-quality, compliant, and participant-focused care. The Social Services Supervisor leads a multidisciplinary social services team and plays a critical role in care planning, staff development, regulatory compliance, participant advocacy, and collaboration with community partners.
Under the direction of the Center Manager PACE/ADH, the Social Services Supervisor actively participates as a member of the Interdisciplinary Team (IDT), contributing to assessments, care planning, intake, discharge planning, and family engagement. This position is ideal for a strong social work leader who is passionate about serving seniors, skilled in supervision, and committed to supporting both participants and staff in a mission-driven environment.
Responsibilities:
- Serve as an active member of the Interdisciplinary Team (IDT), participating in initial, semi-annual, and unscheduled assessments, daily morning updates, intakes, and family meetings.
- Provide leadership and oversight for social work services, including care planning, disenrollment processes, grievances, and ongoing monitoring of participant satisfaction.
- Supervise Social Workers, Social Service Coordinators, and the Activities Coordinator, including hiring, onboarding, training, performance evaluations, timecard approval, PTO and mileage approval, and facilitating regular staff meetings.
- Oversee support services such as government-issued cell phones, SNAP/food stamps, LIHEAP applications, medical supplies, pest control, incontinent supplies, Life Alert systems, escorts, outsourced counseling, and behavior contracts, ensuring quality service delivery and participant engagement.
- Coordinate discharge planning for participants transitioning from hospitals, nursing facilities, respite care, skilled nursing facilities (SNF), or long-term care (LTC), serving as a liaison with internal teams and external providers.
- Maintain accurate, timely, and comprehensive case management documentation, including reassessments, service coordination, and participant preferences.
- Provide individual and family counseling, facilitate group counseling and support activities, offer crisis intervention and advocacy, and oversee family mediation in collaboration with the IDT.
- Plan, provide, and coordinate in-service education and ongoing training opportunities for staff and families.
- Participate in community-based participant conferences and represent LIFE PACE at community events as assigned.
- Oversee financial coordination related to outsourced counseling, participant financial contracts with the State/DHS, and collaboration with PACE Finance on eligibility and recertifications.
- Ensure implementation of PACE policies and procedures and participate in quality improvement (QI) activities as assigned.
- Maintain compliance with HIPAA and HITECH regulations and complete all mandatory annual training.
- Attend and actively participate in staff meetings, committees, special projects, and organizational initiatives.
Requirements
Qualifications:
· MSW and/or LCSW required.
· Minimum of one (1) year of documented experience in working with senior adult population required.
· Prefer experience in a community-based setting or geriatric program.
· Prefer three (3) years of supervisory experience.
· Holds and maintains a valid Oklahoma driver's license, proof of vehicle insurance, clean driving record, and use of personal vehicle.
· Spanish bilingual skills strongly preferred.
Knowledge, Skills & Abilities:
· Interest in the risk-based long-term care program to serve frail elderly in a community-based setting.
· Experience and thorough knowledge of social service principles and practices.
· Knowledge of psychosocial, behavioral, and family needs of the elderly population.
· Experience in supervising staff.
· Knowledge of financing mechanisms such as Medicare and Medicaid.
· Knowledge of the Tulsa County health and social service delivery systems and aging network.
· Proven ability to work in an interdisciplinary team.
· Ability to work effectively and harmoniously with the staff, the elderly, providers of services, and public and private agencies.
· Effective oral and written communication skills.
