Physical Therapist

Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient care across home health, and community care. This role involves developing personalized care plans, implementing physical therapy techniques, and collaborating with multidisciplinary teams to promote recovery and improve patient outcomes. A strong background in anatomy, physiology, medical terminology, and specialized areas such as orthopedics, geriatrics, pediatrics, and trigger point therapy is essential. The Physical Therapist will play a vital role in discharge planning and ensuring seamless transitions of care for diverse patient populations.

Duties

• Assess patients' physical conditions through detailed evaluations of mobility, strength, and functional abilities.
• Develop and implement individualized care plans tailored to patient needs across various settings such as home & community care.
• Perform therapeutic interventions, including physical therapy, trigger point therapy, and other modalities to facilitate recovery.
• Educate patients and their families on exercises, injury prevention, and health maintenance strategies.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate effective treatment plans.
• Manage discharge planning to ensure continuity of care post-treatment.
• Document patient progress accurately using EMR systems and maintain detailed medical records in compliance with healthcare regulations.
• Stay updated on the latest advancements in orthopedics, geriatrics, and other relevant specialties to enhance treatment efficacy.
• Participate in ongoing professional development activities to maintain licensure and expand clinical expertise.

Experience

• Proven experience in inpatient, outpatient, hospital, or clinic settings with a focus on patient care.
• Demonstrated expertise in managing diverse populations including geriatrics, and patients requiring post-surgical or orthopedic rehabilitation.
• Familiarity with discharge planning, care plans, and working within multidisciplinary teams.
• Experience with home health or home & community care is highly desirable.
• Proficiency in using EMR systems for documentation and treatment planning.
• Knowledge of physiology, anatomy, and relevant medical terminology necessary for accurate assessment and treatment.

Pay: $ $75.00 per hour

Expected hours: 10 – 12 per week
