Summary of the Position: The RN Wound Care Nurse is responsible for the comprehensive assessment, treatment, documentation, and management of residents with altered skin integrity in the long-term care setting.  This role ensures compliance with CMS regulatory requirements including F-Tag 686 (Treatment/Services to Prevent/Heal Pressure Ulcers) and promotes evidence-based practices to prevent skin breakdown and support wound healing. 

Work hours:  Monday-Friday 8:00 am - 4:00 pm

Job Responsibilities:

  • Conduct comprehensive initial and ongoing wound and skin assessments for all types of wounds.
  • Accurately stage pressure injuries and identify wound etiology in accordance with current clinical guidelines.
  • Develop, implement, and revise individualized wound care treatment plans based on resident needs and physician orders.
  • Provide or oversee wound treatments, including advanced dressings and treatments.
  • Monitor wound healing progress and promptly communicate changes to providers.
  • Ensure accurate, timely, and thorough wound documentation, including measurements and progress notes.
  • Collaborate with MDS staff to ensure accurate coding and documentation related to skin integrity.
  • Lead pressure injury prevention initiatives, including risk identification and preventive interventions.
  • Collaborate with interdisciplinary team members including nursing, therapy, dietary, and providers.
  • Educate and support nursing staff and CNAs on wound prevention, identification, and treatment.
  • Participate in quality assurance and performance improvement activities related to sin integrity.
  • Assist with survey preparation and respond to wound-related survey inquiries.

Survey and Regulatory Compliance – F-Tag 686

Ensure residents who enter the facility without pressure injuries do not develop them unless unavoidable, and that residents with pressure injuries receive necessary treatment and services to promote healing, prevent infection, and prevent new pressure injuries from developing.  Maintains documentation that clearly supports clinical decision-making, interventions, monitoring, and outcomes.

Qualifications

Education and Experience Requirements:

Current, active West Virginia registered Nurse (RN) license in good standing.

Minimum of 1-2 years of nursing experience in long-term care or skilled nursing.

Wound care certification (WCC, CWCN, or equivalent) preferred or willingness to obtain.

Strong knowledge of CMS regulations, skin integrity standards, and evidence-based wound care.

Physical Demands:

The employee must be able to communicate professionally with residents, families, and facility staff.
