Lakeview Village is a continuing care retirement community. We have an opening for a Home Health Intake & Scheduling Coordinator to work Full Time.

Lakeview's Home Health Intake & Scheduling Coordinator provides administrative support and intake, scheduling, and clerical services to the Home Health Administrator/Director of Nursing in order to increase the effectiveness and efficiency of the Home Health department.

Job Duties and Responsibilities

  • Provides general administrative duties for the Home Health division including, but not limited to: answering phones, taking messages, managing fax communications, managing physician orders and referral calls, and creating, modifying, and copying forms.
  • Works closely with the Admissions Liaison and Registered Nurse Case Managers to facilitate staffing in accordance with resident needs.
  • Provides the Home Health Administrator with computer support for certifications, re-certifications, and agency admissions.
  • Assists with the distribution of Medicare paperwork as directed, tracks physicians' orders, assembling and disassembling medical charts, quality assurance activities, and regulatory compliance.
  • Assists with departmental purchasing requests.
  • Supports, expands, and maintains marketing efforts to promote departmental programs and services.
  • Coordinates and assists with projects and meetings as needed.
  • Responsible for the central supply system – checks for par level, stocks shelves, and orders supplies.
  • Responsible for the Home Health medical records system in regards to admissions and discharges.
  • Responsible for ordering office supplies and coding all invoices for Home Health.
  • Develops and maintains a good working rapport with inter-department personnel, other departments within the facility, and off-campus contacts to ensure that departmental programs can be properly maintained to meet the needs of the facility and residents.
  • Ensures that Residents' Rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to express grievances are followed.
  • Maintains confidentiality of sensitive information.
  • Handles stress well, gets along with others in the workplace, maintains acceptable standards of conduct.
  • Adheres to all safety regulations and requirements.
  • Completes trainings as required by policy.
  • Supportive of Lakeview Village's vision, mission, and values.
  • Performs other duties as assigned.

  • High School Diploma or GED is required.

  • At least one year of experience in scheduling and intake coordination for Home Health and Private Duty is required.
  • Must demonstrate a good working knowledge of the Microsoft Office Suite software. Must adapt quickly to new systems.
  • Demonstrates good communications skills, including but not limited to effective reading, writing, speaking, and analytical skills, with a fluent understanding of English.
  • Ability to work independently, prioritize tasks appropriately, and exercise sound judgement where procedures cannot be standardized.
  • Ability to administer and follow straightforward written or verbal instructions.
  • Demonstrates both precision and accuracy in the completion of duties each day.
  • Possesses an empathetic approach to the elderly.
  • Able to work in a team setting with residents, relatives, staff, and others.
