Case Manager - Senior Outreach and Engagement Description:
Senior Outreach and Engagement is intended to serve at risk older adults (age 55 and older). The goals of the program are to assist older adults to remain in their community and to improve their quality of life. This is accomplished by linking to community programs and resources, care coordination with community providers, medical care management and coordination, engagement, assessment, case management, education, support, linkage to behavioral health and/or substance abuse treatment. Services are provided in the home and community. The case manager provides person-centered and strength based care.
Responsibilities:
- Provide services and conducting daily business and interactions with clients, staff outside contacts and referral sources with positive and professional customer relations.
- Provide assessment of assigned clients community services needs.
- Develop, monitor, and update the Community Service Plan with the client.
- Develop and maintaining required relations with staff and other service programs for the purpose of obtaining services.
- Initiate client involvement in the agreed upon services, including monitoring and follow through to assure appropriateness and continuity of the services.
- Provide individual or group counseling, education, support, transportation and daily living skill development services to assigned clients to enhance client's independent functioning.
- Provide advocacy and ensuring continuity of care for assigned clients.
- Provide accurate, appropriate and complete records, documentation and reports.
- Perform other duties as assigned.
Schedule:
Monday through Friday 8:30 A.M. - 5:00 P.M.
Qualifications:
- Bachelor's Degree in relevant human services field or Associate's Degree in relevant human services field plus two years of relevant experience.
Salary/Hourly Range:
$ $21.50 hourly
Bilingual (Spanish/English) rate of pay: $ $22.59 per hour (depending on experience)
