POSITION SUMMARY
The Resident Care Coordinator is an RN who has developed managerial and clinical skills through experience and advanced training. Assists in the planning, development and implementation of processes and systems within the department. This position is responsible for the coordination of day to day clinical operations with licensed and non-licensed personnel who provide health care to residents and with the interdisciplinary team. Provides direct patient care as necessary.

A
Manages day to day operations of SNF and assists in the design, implementation and evaluation of nursing systems that ensure consistent delivery of resident focused care and the promotion of resident rights.

Assists MDS Coordinator with administrative responsibilities of both departments in the absence of the Director of Nursing
C
Promotes positive team relations by maintaining effective communication with all staff, other departments, physicians, residents, families and visitors
D
Continually assesses the quality and appropriateness of resident care by conducting resident rounds and facilitating the resolution of resident concerns and complaints as they arise
E
Applies leadership qualities in daily activities using problem solving and conflict resolution skills effectively in regards to all staff, residents, families and visitors
F
Assists in the planning and directing of team meetings. Attends nursing management meetings in the absence of the DON
G
Assists in the development, revision, and review of departmental policies and procedures assuring they meet current practice standards
H
Maintain current knowledge of all regulatory standards. Demonstrates accountability for nursing practice through continuous professional growth and development compliance with state and federal regulatory agencies

Assists in the admission and discharge planning processes

Identifies and resolves problems related to delivery of resident care
K
Assists to recruit, select, orient, and train staff to meet the needs of the department
L
Collaborates with DON to address and resolve resident, family, physician, and staff concerns/complaints
M
Displays fiscal responsibility and assists with the development of annual capital and operational budgets
N
Promotes harmonious relationship and open line of communication within the department, with the IDT and other support personal to ensure quality of care

Maintains confidentiality of all residents/staff information
P
Participates in all facets of department quality improvement and infection control processes, including EOC rounds and SNF safety meetings

Assists the DON with the development, implementation of all newly mandated Federal and State regulatory initiatives
R
Reviews monthly pharmacist report, follows up on provider issues and resolutions
S
Audits physician progress notes and coordinating with providers to maintain timelines of documentation

Participates in developing and reviewing care plans

Arranges for resident's diagnostics and therapeutic services/appts/outings

Assists and reviews potential SNF admissions

Attends nursing daily report routinely

Orientation of new staff onboarding on unit
Y
Cerner super user ability, run reports in care tracker where needed
Z
Keep Caretracker current for resident documentation
Accurate, objective, and professional documentation in HER for tasks completed as RCC
Coordinate all preparation for appointments and outings medical or person, including communication tool to staff., communication with POA/guardian/family, provider, outpatient provider, pharmacy, and any other areas to provide for safe and appropriate time away.
Coordinate all medical referrals
Coordinate for any planned surgery
Coordinate all medication annual renewals with pharmacy/provider
Assist in emergent or urgent transfers of residents
Collaboration with unit provider related to outings/appointments, may include forwarding of notes to provider in Cerner
Assist with specialty orthopedic devices for residents and implementation of such devices
Coordinate all items needed for telehealth appointments
Assist with management of supplies on hand and new supplies that are needed
Working knowledge of all supplies or furniture used for residents/care
Coordinate with all entities when sleep apnea devices are needed
Management and oversight of wound vac, purchase orders of, and supplies for any wound vac
Assist with any postmortem processes- facilitate personal supplies or belongings of residents and family
Coordinate of family/caregiver conversation, including use of work cell phone/three-way calls/in person meetings
Cohesive real time problem solving for issues with medications/lab/radiology/nursing unit
Assist with specialty medication process
Oversight of rail process/bed against the wall ongoing protocols
Active participation with QAPI program – including ongoing PIPs
Weekly count of all unit Omnicell and assist with trust fund monies processes
Active participation in weight IDT team meetings, oversight of collection of monthly weights
Actively preparing for survey at all the times
Assist and present to any surveyors during state survey process
Other related duties as assigned.
Qualifications
POSITION QUALIFICATIONS
MINIMUM EDUCATION: High school diploma
Graduated from an accredited school of nursing as an RN
PREFERRED EDUCATION:
MINIMUM EXPERIENCE: Minimum of 3 years experience in a long term, skilled or dementia care facility
REQUIRED CERTIFICATIONS: Valid Nevada RN license with evidence of ongoing education towards RN licensure
BLS
PREFERRED CERTIFICATIONS/LICENSES: RAC-CT, Certified Dementia Care Practitioner, Certification in Geriatric nursing and experience in a leadership role
SPECIAL SKILLS:
SUPERVISES: Delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties
