We are seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health and Hospice team serving patients in the East Bay Area. In this role, you'll work one-on-one with patients in the comfort of their homes to help them regain mobility, manage pain, and improve their quality of life. You’ll collaborate with a multidisciplinary team to develop and implement individualized care plans.
*Key Responsibilities:*
* Conduct in-home physical therapy evaluations and assessments
* Develop and implement individualized treatment plans based on patient needs and goals
* Educate patients and caregivers on therapy exercises and safe mobility techniques
* Monitor and document patient progress in accordance with agency and regulatory guidelines
* Coordinate care with nurses, occupational therapists, physicians, and other healthcare professionals
* Provide ongoing updates to the clinical team and revise treatment plans as needed
* Ensure compliance with all state, federal, and agency policies and procedures
*Requirements:*
* Current California Physical Therapist (PT) license
* Graduate of an accredited Physical Therapy program
* Valid Driver’s License and reliable transportation
* Ability to work independently and manage a flexible schedule
* Strong communication and documentation skills
* CPR certification (BLS)
*Preferred Qualifications:*
* Experience with OASIS documentation
* Bilingual (Spanish/English) a plus
* Minimum 1 year of experience preferred, ideally in home health or geriatrics

The compensation range for this position *is $65.00 to $80.00,* commensurate with experience and qualifications. Care Must also offers a comprehensive benefits package, including health, dental, vision, PTO, paid holidays, and flexible spending accounts.
