Position Overview
Our Physical Therapists will deliver individualized, evidence-based therapy to support patient recovery and maximixe mobility. They will evaluate patient needs, develop individualized treatment plans, and implement therapeutic activities tailored to each patient's needs and environment. This role involves close collaboration with patients, families, and interdisciplinary teams to ensure safe, effective, and goal-oriented care. Key Responsibilities:

• Assessment & Evaluation

Conduct comprehensive physical assessments, review medical history, and identify functional limitations to develop individualized care plans.

• Treatment Planning & Intervention

Design and deliver evidence-based therapeutic exercises, manual therapy, modalities, and rehabilitation programs tailored to patient goals.

• Patient Education & Advocacy

Equip patients and caregivers with instruction on home programs, injury prevention, adaptive strategies, and lifestyle modifications to support progress.

• Interdisciplinary Collaboration & Documentation

Communicate with physicians, nurses, and other care providers while maintaining timely, accurate clinical documentation and progress reports.

• Outcome Management & Quality Improvement

Monitor patient outcomes, adjust plans based on response, and participate in quality assurance activities to drive positive patient results and compliance with standards.

What We're Looking For

• Current license in the state of Colorado
• At least 1 year of home health care Physical Therapy experience
• Strong interpersonal and critical thinking skills
• Compassionate, patient-centered mindset
• Reliable transportation and valid driver's license
