Licensed Practical Nurse

Job Summary:

• Title: Licensed Practical Nurse - Rehab
• Location: Rio Rancho, NM 87124
• Duration: 8 weeks (Extending)
• Shift: 7:00 PM to 7:00 AM. 12 hours. 3 days a week.
• Pay rate: Lucrative pay + Benefits + Bonus.

Requirements:

• Must have 2+ years of recent Inpatient rehab & hospital experience.
• Active license in NM state.
• COVID Vaccinated.
• BLS / CPR Preferred.

Responsibilities :

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
• Administer medications, injections, IV infusions, and other treatments following physician orders and safety protocols.
• Assist with complex procedures such as catheterization, wound suturing, tube feeding, and sterile processing techniques.
• Manage patient care plans by coordinating with multidisciplinary teams for discharge planning, case management, and post-acute care.
• Provide specialized care for patients with conditions like Alzheimer's disease, dementia, infectious diseases, or those requiring hospice or palliative support.
• Conduct patient triage in urgent care or emergency room settings to prioritize needs based on acuity levels such as Level I Trauma Center or Level II trauma experience.
• Maintain accurate medical documentation adhering to HIPAA regulations and ICD coding standards (ICD-9/10).
• Support infection control practices and ensure aseptic techniques during all procedures to promote safety for patients and staff.
• Educate patients and their families on health coaching topics including medication administration, nutrition, pain management, and chronic disease management.
• Collaborate with physicians in specialized areas such as cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), or neonatal intensive care (NICU experience).
