The Part-Time Families Matter Case Manager provides community-based intervention, brief case management, and evidence- rmed parenting education to parents and expectant parents impacted by substance misuse. This role supports mothers, fathers, and caregivers with children up to age six by strengthening protective factors, reducing risk, and promoting healthy family functioning. The Case Manager delivers parenting-focused education while connecting families to critical resources across the community.

WHAT YOU WILL BE DOING:

  • Works under the supervision of the Director of Intervention Programs.
  • Part-time role; ~25 flexible hours per week, with some evenings required for classes, enrollment, case management sessions, and alternative activities.
  • Deliver evidence-informed parenting education through group classes and one-on-one sessions.
  • Assist the Director of Intervention Programs with quality management procedures to evaluate PADRES program effectiveness, performance goals, and outcome measures.
  • Read, understand, and adhere to the PADRES Statement of Work; seek clarification as needed to ensure compliance.
  • Provide quarterly alternative activities that promote healthy lifestyles, parenting skills, and family bonding.
  • Contact participants referred by the Department of Family and Protective Services (DFPS) within three (3) business days.
  • Provide case management services, including referrals to OSAR, DFPS, substance use treatment, and other community resources based on assessed needs.
  • Conduct outreach presentations and screenings for parents with children under six and/or expectant parents.
  • Maintain complete and accurate caseload documentation in hard copy client charts and required electronic systems (including CMBHS), ensuring all signature requirements are met.
  • Submit monthly reports, performance measures, timesheets, and mileage documentation in accordance with agency guidelines.
  • Attend required meetings, including the Annual System Agency Neonatal Abstinence Syndrome Conference.
  • Assist with other duties as assigned by the Director of Intervention Programs.

WHAT IS REQUIRED:

Must meet one of the following minimum qualifications:

Associate degree or higher in a behavioral health–related field; or

Two (2) years of experience in a behavioral health field, including substance use disorders; or

Qualified Credentialed Counselor (QCC); or

Appropriately supervised credential or license, such as LCDCI, LCDC, LPC, LMFTA, CCJP-A or similar.

  • Strong organizational, documentation, and time-management skills.
  • Proficiency in computer systems and willingness to learn CMBHS and virtual learning platforms.
  • Ability to work independently with minimal supervision.
  • Must be able to lift up to 50 lbs and work evenings as needed.
  • Reliable transportation, valid driver's license, and auto insurance required.
  • Must pass background check and drug test.

PREFERRED QUALIFICATIONS:

  • Experience providing case management, parenting education, or intervention services.
  • Experience working with parents, expectant parents, or families affected by substance use.
  • Knowledge of informed care, harm reduction, motivational interviewing, and stages of change.
  • Familiarity with community resources related to behavioral health, basic needs, and family support.
  • Strong interpersonal, oral, and written communication skills.
  • Ability to plan, organize, prioritize, and meet deadlines in a community-based setting.

TRAINING REQUIREMENTS:

  • Complete all PADRES-required trainings within the first 90 days of hire, including curriculum certifications, informed care, harm reduction, prenatal and parenting education, child abuse prevention, and substance use–related trainings.
  • Complete required annual continuing education hours as specified by the System Agency.
  • Maintain documentation of all completed trainings for review upon request.
