The Forest at Duke is an upscale retirement community with a beautiful new Health Center Staffing ratios of 1 CNA to 5 residents & 1 Nurse to 20 residents

The Nurse Care Manager directs and provides person-centered, rehabilitative, holistic care and chronic disease management to residents. The Health Center follows the Small House Model of care-- a person-centered approach where residents have a significant say in their daily lives, their care, and their living environment.

This position works three 12-hour shifts which include every other weekend. Competitive pay up to $32.75/hour for LPNs and $38.00/hour for RNs.

We are hiring for:

Full Time- 3/12-hour shifts: 7:00pm – 7:30am (Nights) or 7:00am – 7:30pm (Days)

Job description

Join our Small House team and enjoy providing individualized care in a homelike setting The Forest at Duke is thrilled about the recent opening of our newly-constructed, state of the art, Health Center. Utilizing the Small House concept, the new Health Center features nine "small houses" that support varying levels of care within our established continuum: Assisted Living, Memory Care, and Skilled Nursing.

The RN/LPN directs and provides person-centered, rehabilitative, holistic care and chronic disease management to residents in household. The licensed team members provide leadership for all care partners in their assigned houses and ensure that care is provided as developed by the individualized care plan. They have oversight and supervisory responsibility for day-to-day nursing care and programs and services provided by the neighborhood care team in compliance with current federal, state, and local standards governing the facility.

The position works three 12 hour shifts, including every other weekend.

Key Responsibilities:

  • Responsible for supervising the care team for their assigned house(s), including staffing, work assignments, performance evaluation and improvement, coaching and training, disciplinary actions and investigations, injury reporting, and problem-solving.
  • Provide care to meet resident needs including direct care, medication administration, and treatments.
  • Conducts clinical rounds on all assigned resident every shift.
  • Makes rounds with physicians as necessary and ensures proper record keeping. Consults with physicians in providing care, treatment, diagnostic and therapeutic services, and notifies physicians of resident accidents, incidents, and changes in condition.
  • Collaborates with all members of the interdisciplinary team to assure holistic wellness.
  • Collaborates with the physician and pharmacist to manage resident medications.
  • Conducts assessments, leads care planning, writes and or adjusts the resident's care plan, and manages family expectations for the residents assigned.
  • Complete MDS assessments in EHR. If RN, finalize the process by submitting and locking the assessments.
  • Other RN duties as assigned

Key Requirements:

  • Must be a licensed nurse (RN or LPN) currently licensed by the State of North Carolina, with 3-5 years of experience; geriatric nursing is preferred.
  • At least two years' experience as a Charge Nurse or Nurse Supervisor is required, and must include direct supervisory experience.
  • Must be willing to promote standards of person-centered care.
  • Must have strong computer skills; previous experience with Electronic Health Records is preferred.
  • Must show ability to read, write, and understand English.
  • Must have certification in ServSafe and BLS/CPR. Can be obtained during first 90 days of employment.
  • Must be knowledgeable of nursing and medical assessment practices and procedures and laws, regulations, and guidelines governing long-term care.
  • Must be able to assist in planning, supervising, coordinating, and evaluating the activities of nursing team members.
