We are looking for an experienced Case Manager to join our team at our Intensive Outpatient Program in Dallas
Job Summary:
The Case Manager provides critical support in coordinating care, discharge planning, and external referrals for clients at Discovery MSO. Working as part of a multidisciplinary team, the Case Manager serves as a liaison between the clinical team, families, and outside providers to ensure seamless and effective continuity of care during and after treatment.
Essential Duties and Responsibilities:
- Serve as the primary contact for families and support systems regarding discharge planning and treatment progress.
- Coordinate outpatient referrals for therapy, psychiatry, primary care, support groups, and housing as applicable.
- Develop individualized discharge and aftercare plans based on client needs and clinical recommendations.
- Maintain frequent communication with external providers and referral partners to ensure smooth transitions.
- Participate in multidisciplinary treatment team meetings to track client progress.
- Document case management activities and discharge planning updates in the electronic medical record (EMR).
- Assist with scheduling outside appointments or assessments (e.g., legal, educational, medical).
- Advocate for client needs, identify barriers to care, and help connect them to available resources.
- Educate clients and families about recovery resources, insurance limitations, and ongoing care strategies.
- Ensure compliance with HIPAA, state regulations, and organizational protocols.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Public Health, or related field required.
- 1–2 years of experience in case management, care coordination, or discharge planning.
- Experience in behavioral health, substance use treatment, or residential settings preferred.
- Knowledge of community resources, behavioral health systems, and referral networks.
- Proficiency in EMR systems and documentation standards.
- Strong interpersonal, communication, and problem-solving skills.
Job Type: Full-time; exempt
Pay Range: $40,000 - $54,000 per year
Schedule:
- Day Shift: 9:00am - 5:30pm
- Weekends as needed

Work Environment and Physical Requirements:
- Facility-based position with frequent client interaction.
- Regularly works on a computer and communicates via phone and email.
- May require occasional evening or weekend availability for discharge transitions.
FLSA Classification: This position is Exempt and is not eligible for overtime under federal and state wage and hour laws.
Pay: $40, $54,000.00 per year
