Role Summary
The Physical Therapist is responsible for planning, organizing, and delivering high-quality physical therapy services to patients across a variety of settings. This role focuses on assessment, treatment, education, and collaboration to promote independence, safety, and enhanced quality of life. Duties and Responsibilities

• Provide exceptional physical therapy care as an active and collaborative member of the team.
• Conduct in-person assessments for patients at initial enrollment and as needed, based on team determinations.
• Evaluate and recommend adaptive and durable medical equipment, specialized footwear, and prosthetics; assess home safety and mobility, recommending modifications and equipment as appropriate.
• Design and implement physical therapy treatment programs consistent with professional standards and organizational policies.
• Deliver therapeutic interventions aimed at achieving individualized goals; collaborate with the team on patient care planning.
• Accurately document evaluations, assessments, treatment plans, and outcomes in a timely manner.
• Measure and document treatment effectiveness, revising plans as needed.
• Educate and train patients, families, and caregivers on home programs, techniques, and equipment use to support ongoing recovery and independence.
