Licensed Practical Nurse (LPN) – Wound Care/Home Health

Overview:
Chicago Home Healthcare is looking for an experienced Licensed Practical Nurse (LPN) with proven wound care skills to provide high-quality care to patients in their homes. We are seeking compassionate professionals who excel in clinical care, communication, and documentation.

Responsibilities:

• Deliver in-home skilled nursing services according to individualized care plans
• Perform advanced wound care treatment, including wound assessment, dressing changes, and monitoring for infection or complications
• Document all wound care interventions accurately in the EMR
• Collaborate with RNs, physicians, and other caregivers to ensure coordinated care
• Educate patients and caregivers on wound management and prevention
• Administer medications and treatments as prescribed
• Maintain professional boundaries, confidentiality, and compliance with agency procedures

Requirements:

• Current Illinois LPN license
• Minimum 1 year wound care experience required (home health experience a plus)
• Strong clinical judgment and problem-solving skills
• Ability to travel to patient homes; reliable transportation
• CPR certification
• Excellent communication and patient-education abilities

Pay: $40.00 per hour
