Primary Responsibility: To provide direct social service to youth and families
To help youth adjust to life, to resolve his particular presenting problems, and to make his experience at Nazareth one of continuous physical, academic, mental, social and emotional growth
To assist families in understanding the dynamics of their child's behavior, to strengthen the parent-child relationship and to prepare families in achieving reunification or the most positive contact possible for all members of that family
To provide individual, family and group casework
Essential Duties:
Act as the intake worker and arrange for and support plans for pre-placement visits, ensure all placement agreements are signed at intake, formulate a differential diagnosis and participate in developing an individual service plan for the youth and family
Provide individual therapy and group work for youth, casework services to families, and family therapy where appropriate
Act as educational case manager for youth and attend IEP Team Meetings
Keep regular contact with the youth and/or family; continuous contact with the parents, teachers, and all other persons, in and outside of Nazareth, who have contact with the youth and/or family so that the efforts of all can be coordinated to understand and meet the needs of the youth; act as liaison with the referring agency
Interpret special needs of child and/or family to staff and provide consultation regarding trauma's effect for special needs
Provide crisis intervention services during periods of emotional upset when such services would be helpful
Job Description: Social Worker
Work with the direct care staff on a consistent basis to plan for the implementation of the youth's individual service plan
Interpret casework findings to the staff working with the children and make recommendations for therapy and other services
Work with the youth and family relative to ongoing services and planning as well as for discharge and after–care services
Provide after-care casework service to the child and/or his family
Record contacts with the youth and/or family as specified in the Licensing Regulations promulgated by the Massachusetts Department of Early Education and Care
Maintain records in accordance with agency standards; participate in Quality Assurance Reviews, Utilization Reviews and Gateway Treatment Plans
Provide clinical supervision to student interns and/or selected direct service staff on an as needed basis; responsibilities include orientation and training, case record reviews
Adhere to Agency Policies and Procedures, State Contractor's regulatory requirements, Department of Early Education and Care Regulations and other external regulatory standards
Participate in Licensing and Accreditation Reviews as needed
Provide input to program planning, program development and strategic planning processes as requested/needed
Participate in all mandated training (including MAP) and other training opportunities for required training hours and professional development
Participate in staff meetings and peer supervision
Drive youth to appointments, i.e., medical, dental, sports activities, when necessary
Respect, maintain, and honor confidentiality
Schedule case conferences in conjunction with the Executive Director
Chair case conference treatment meetings in the absence of an administrator
Qualifications:
- Master's Degree in Social Work or related Human Services field with knowledge of Family and Child Welfare service delivery systems
Skills/Abilities:
Ability to view situations, issues, and conditions from a variety of perspectives
Ability to anticipate, identify and articulate program problem areas and to participate constructively in the design of solutions
Ability to develop treatment modalities/skills in direct service case practice
Ability to be flexible, sensitive, supportive of the needs of others, and reliable; able to balance competing demands of ongoing work and occasional emergencies; identify needs for assistance and calls for same when appropriate
Ability to communicate and coordinate with other program staff within agency; willing and able to work as a team member
Ability to learn from mistakes and able to help staff do the same
Ability to follow written and oral instructions
Ability to prepare comprehensive, articulate, and legible reports
Licenses/Certifications:
Licensed and/or certified by the Commonwealth of Massachusetts
Required to renew licenses and/or certificates at the time of expiration; responsible for ensuring that all continuing education unit requirements are met within the allotted time frame
Must demonstrate maturity and ability to deal effectively with emotionally demanding and stressful situations; must demonstrate good judgment in decision making and determination of information that should be communicated to staff and other supervisors
Must continue to achieve a satisfactory annual competency and performance evaluation
- Licensed or "licensed ready" preferred
