*Responsibilities*
* Assess patients' physical abilities and limitations through comprehensive evaluations.
* Develop individualized treatment plans based on patient needs and goals, incorporating evidence-based practices.
* Implement therapeutic exercises and modalities to improve mobility, strength, and overall function.
* Educate patients and their families about treatment plans, home exercises, and self-management strategies.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care and effective discharge planning.
* Maintain accurate documentation of patient progress, treatment interventions, and outcomes in compliance with regulatory standards.
* Stay updated on advancements in physical therapy practices.
*Qualifications*
* Masters (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program.
* Valid state licensure or eligibility for licensure as a Physical Therapist.
* Strong knowledge of medical terminology related to physical therapy practices.
* Experience in skilled nursing settings is preferred but not required.
* Familiarity with discharge planning processes to facilitate smooth transitions for patients.
* Background in treating geriatric populations is a plus.
* Excellent communication skills to effectively interact with patients, families, and healthcare professionals.

Pay: $45.00 - $55.00 per hour
