*Occupational Therapist (Per Diem – Travel Home Health)*
_This is a travel position providing home health services within Davis and Woodland. Home health experience is __*not required*_ _— we provide full training and support to help you succeed._
*Essential Functions:*
* Coordinate patient care and communicate effectively with physicians, patients, family members, and the Home Health & Hospice interdisciplinary team.
* Provide occupational therapy interventions and modalities appropriate to the patient’s clinical condition and individual needs.
* Assess patients’ needs for durable medical equipment (DME) and adaptive equipment, and make appropriate recommendations.
* Evaluate and address patient pain, incorporating occupational therapy–based pain management strategies as appropriate.
* Continuously assess and reassess patients’ clinical status and provide, modify, or discontinue occupational therapy services in accordance with physician orders.
* Participate in interdisciplinary team meetings and care conferences in accordance with agency policy.
*Qualifications:*
* Graduate of an approved and accredited Occupational Therapy program.
* Active, current Occupational Therapist license in the state of California.
* Valid driver’s license, current vehicle insurance, and access to a reliable vehicle.
* Current Basic Life Support (BLS) certification through the American Heart Association.
* Current health certificate/physical examination and TB clearance, as required.
* *Home health experience is preferred but not required. Full training will be provided.*
* Ability to successfully pass a criminal background check, drug/alcohol screening, and OIG exclusion screening.
