Social Worker/Auditor

We are looking for a person who loves details The social worker position also has an auditing component where all IPCs are reviewed by the auditor before submission.

RESPONSIBILITIES AND DUTIES:

1) Interview and screen all referrals to determine the appropriateness of the prospective participant for a full ADHC/CBAS assessment. Provide referrals for applicants who are not appropriate for ADHC/CBAS.

2) Responsible for completing initial assessments for every participant and assess the psychosocial needs of the participants as a member of the multidisciplinary assessment team. Develop an individualized plan of care for each participant including short term and long term goals. The participant's family and other appropriate individuals will participate in developing the plan of care.

3) All assessments will expressed by participant/family members/caregiver/legal representative and their needs, goals, and desired outcome, using their own words, and direct quotes in the development of the person-centered planning/IPC.

4) The social worker is required to provide all services as specified on the IPC and document the reactions to those services on the same day the services are provided. Maintain signed and dated documentation of all services performed on the day of service and signed and dated quarterly progress notes.

5) Make referrals to available community resources. Maintain current roster of community resources.

6) Develop peer support groups and family caregiver groups

7) Develop and facilitate problem oriented discussion groups for participants.

8) Serve as liaison to the participant's family and other caregivers.

9) Serve as liaison to community agencies providing services to the participant.

10) Participate in community education, networking and advocacy groups serving frail and disabled older adults.

11) Provide discharge planning for all participants discharged; and assume responsibility for implementation and coordination of the discharge plan.

12) Conform to all applicable policies of the Center and attend the orientation program offered by then Center.

13) Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies, and monitor staff compliance.

14) All initial, 3 month, 6 month and any needed assessment/reassessment for each participant.

15) Must complete reassessments at least every six months and as the case may be per participant, a reassessment may need to be completed more often than every six months due to a change in the participant's status, a participant's length absence, following hospitalization, etc.

16) Must provide in-service training to the participants and/or staff.

17) Must supervise the social work assistant(s).

QUALIFICATIONS:

  1. The Social Worker must either:

a) hold a master's degree in social work from an accredited school of social work;

b) hold a master's degree in psychology, gerontology, or counseling from an

accredited school and has one year of experience providing social services in one or more of the fields of aging, health, or long-term care services;

c) The person is licensed by the California Board of Behavioral sciences; or

d) hold a bachelor's degree in social work from an accredited school with two

years of experience providing social services in one or more of the fields of aging, health, or long-term care services.

  1. Experience and knowledge of medical social work scope of practice and knowledge of the special needs of functionally impaired older adults.
