About the Role 


 The Assessor will have the unique opportunity to work with our Client Services team. They will be responsible for performing the initial assessments of new clients and reassessments of current clients. The Assessor plays a critical role in maintaining the proper client document files and performing office tasks for Meals on Wheels. 

Main Responsibilities 

  • Conduct home/telephone visits to determine clients' ability to consume and store meals as well as check for staff's safety when delivering a meal.
  • Conduct thorough and accurate client assessments and reassessments during telephone and home visits.
  • Ensure that electronic documentation of assessments is in the proper location for easy retrieval during daily operations, monitoring, or audits.
  • Determine eligibility of potential clients for nutrition services. 
  • Provide nutritional information and referrals to clients, working closely with IM's Client Services Team.
  • Report client abuse or neglect to state authorities to the Department of Family and Protective Regulatory Services as well as appropriate internal contacts.
  • Complete every assessment and re-assessment that is assigned, on a timely basis.
  • Assist with large filing projects and perform other duties as assigned by supervisor.
  • Submit timely weekly and monthly reports of productivity to Data/Billing department and to Manager of Client Services. 
  • Attend weekly team meetings and report any issues of service or complaints to appropriate departments.

Qualifications, Skills & Experience

  • Bachelor's Degree in Social Work preferred. 
  • Bilingual in Spanish desirable.
  • One year of client case work experience preferred.  
  • Enjoy working with senior citizens.  Experience working with low-income elderly a plus.
  • Must be able to maintain a work pace adequate to keep all computerized and paper records complete and accurate in a timely fashion.
  • Excellent interpersonal skills and must be able to communicate effectively verbally and in writing.
  • Ability to prioritize tasks and manage time.
  • Skilled in working independently under considerable pressure and without close supervision while maintaining effective and accurate performance.
  • Proficient in Microsoft Office Suite (Word, Excel and Outlook). 
  • Reliable automobile, valid auto registration and inspection, and valid driver's license and automobile liability insurance required.

This position requires driving to client sites five days per week. Having reliable transportation will be essential for this job. Sitting while driving, standing, walking on uneven surfaces, and bending will be done on a daily basis. Seeing, hearing, and talking will be done constantly. Client visits will require getting out of a car to visit client homes under different weather conditions (rain and high degree temperatures during summer). Client homes may have animals or lack temperature control.
