*Occupational Therapist - Care Must*
We are seeking a compassionate and autonomous *Occupational Therapist* to join our *Home Health* team in Monterey, CA. You will provide in-home evaluations and therapeutic interventions that help patients regain independence in daily living activities following injury, illness, or hospitalization. Your role will include assessing safety, mobility, and self-care capabilities in the home environment and delivering personalized interventions.
*Key Responsibilities*
* Conduct comprehensive in-home assessments of patients' physical, cognitive, emotional, and environmental needs.
* Develop and implement individualized care plans to improve function in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
* Provide hands-on therapeutic activities ,including upper extremity rehab, fine motor skill development, energy conservation techniques, and cognitive strategies.
* Monitor progress and document clinical notes, treatment goals, and outcome measures in compliance with CMS, Medicare, and agency requirements.
* Maintain a consistent and timely documentation schedule using EMR (electronic medical records).
* Travel to patients’ homes throughout Monterey and the surrounding areas.
* The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
* Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient’s progress.
* Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
* Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
* Observe, record, and report the patient's response to treatment and changes to the patient's condition.
* Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate to assist the patient.
*Qualifications*
* Master's or Doctorate' in Occupational Therapy from an accredited program.
* *Current California OT license* in good standing.
* At least 1 year of experience in occupational therapy (home health experience preferred).
* Valid California driver’s license and reliable transportation.
* CPR/BLS certification (AHA or equivalent).
* Experience with older adults, post-surgical patients, or those with neurological, orthopedic, or chronic conditions is a plus.
* Strong ability to work independently in the field and manage a flexible caseload.
* Ability and enthusiasm in working with all ages.
*Preferred Skills*
* Familiarity with Medicare documentation and OASIS (Outcome and Assessment Information Set) assessments.
* Experience using EMR systems like Kinnser, Homecare Homebase, or WellSky.
* Excellent communication and interpersonal skills with a patient-centered approach.
* Knowledge of community resources and referral systems in the Monterey area.
