Summary
The Care Management Assistant (CMA) provides support to members of the Care Management team. As a member of the discharge planning team participates in care coordination and transition planning efforts. The CMA supports efficient movement of patients to post-acute facilities including Skilled Nursing Facilities (SNF), Long-term Acute Care Hospitals (LTACH), Inpatient Rehab Facilities (IRF) and Intermediate Care Facilities (ICF) by receiving task triggers to create referrals; transmit to post-acute care provider facilities via HIPAA secure platform and triggering facility pre-certification initiation, or initiates pre-certification as directed.

The CMA supports department compliance to federal and state regulations by serving patient rights notifications related to Discharge Planning and Utilization Management. The CMA works cooperatively with internal interdisciplinary teams, providers, allied health staff, as well as patients, caregivers and visitors. At the direction of the RN Case Manager, Utilization Management RN, and Social Worker, the CMA assists with transitions of care to facilities or arranging follow-up in the outpatient setting.

The CMA works collaboratively and effectively with external community providers/agencies, facility admissions staff and insurance companies. The CMA position requires the individual to take initiative, be proficient, and have strong decision-making skills, organizational skills, communication skills and a commitment to quality and excellence in service.

Minimum Qualifications
Formal Education Required

  • Associate degree or equivalent combination of education and/or experience.

**Experience & Training Preferred

  • Basic experience utilizing Microsoft Office products
  • Knowledge of medical terminology
  • One (1) year experience working with electronic health record (EHR)
  • Previous insurance and prior authorization experience

  • Maintains confidentiality of all patient information
  • Demonstrates basic computer skills as well as navigation and application of electronic devices and platforms
  • Knowledge of CPT and ICD-10 codes
  • Requires interpersonal skills sufficient to interact effectively with patients, their families, interdisciplinary caregivers, peers, subordinates, and supervisors.
  • Excellent verbal and written communication skills
  • Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity

Level of Physical Demands**

  • Sedentary, exerts up to 10 lbs of force occasionally and/or a negligible amount of force frequently.

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay.
