Physical Therapist

Job Summary: Padua Community Services, an intermediate Care Facility (ICF), is seeking a licensed, part-time Physical Therapist to provide assessments and physical therapy services to individuals with intellectual and developmental disabilities in a resident care setting. The Physical Therapist will assess, plan, implement with physician orders, individualized treatment plans.

Direct Report To: Director of Patient Care

Supervises: No

FLSA STATUS: Exempt

Duties/Responsibilities

Essential Job Functions:

Physical Requirements: Ability to assist with transfers, ambulation and therapeutic activities.

Ability to stand, walk, bend, lift and perform physical tasks necessary to provide therapy services.

Minimum Educational and General Qualifications:

Doctor of Phyical Therpay (DPT) degree from an accredited physical therapy program.

Or Bachelor's or Master degree in physical therapy.

Current, unrestricted Physical Therapist license in the State of Louisiana.

Experience providing services to individuals with physical and developmental disabilities.

Ability to work independently while collaborating with an interdisciplinary team.

Strong communication and documentation skills.
