Responsibilities

• Assist clients with activities of daily living (ADLs) such as bathing, grooming, dressing, and mobility.
• Provide emotional support and companionship to promote socialization.
• Monitor clients' health status, including vital signs and overall well-being.
• Support medication administration and adherence to care plans.
• Maintain accurate documentation of services provided in compliance with HIPAA regulations.
• Assist with meal preparation, light housekeeping, and transportation as needed.
• Implement individualized support plans tailored to each client's needs.
• Ensure a safe environment by adhering to safety protocols and emergency procedures.

Qualifications

• Proven experience in home health, assisted living, or working with individuals with developmental disabilities.
• Knowledge of first aid, CPR, and HIPAA regulations.
• Compassionate attitude with excellent communication skills.
• Ability to perform physical tasks including heavy lifting when necessary.
• Experience with patient observation, medication administration, and developing care plans.
• Familiarity with dementia, Alzheimer's, autism spectrum disorders, or memory care is a plus.
• Valid driver's license preferred for transportation responsibilities.
• High school diploma or equivalent required.
