Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our home health care team. The ideal candidate will provide comprehensive patient care in a home setting, utilizing their expertise in various medical procedures and assessments. This role offers the opportunity to work with diverse patient populations, including those requiring long-term care, hospice services, and acute medical interventions. The LPN will collaborate with interdisciplinary teams to ensure high-quality, compassionate care while adhering to HIPAA and other regulatory standards. Experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth is preferred, along with a strong background in patient assessment, medication administration, and infection control.
Responsibilities
- Conduct thorough patient assessments including vital signs, physical examinations, and health history documentation
- Administer medications and injections accurately following physician orders and protocols
- Perform IV infusions, catheterizations, and sterile procedures with aseptic technique
- Manage airway care, ventilator support, and tube feeding for patients with complex needs
- Document all patient interactions and treatments accurately within EMR/EHR systems such as Epic or Cerner
- Provide wound care including suturing and dressing changes in accordance with best practices
- Assist with diagnostic evaluations such as laboratory specimen collection/processing and basic medical imaging procedures
- Educate patients and families on disease management, medication adherence, nutrition, and health coaching
- Coordinate discharge planning and case management for post-acute or outpatient care settings
- Ensure infection control protocols are strictly followed to prevent healthcare-associated infections
- Collaborate with multidisciplinary teams across settings including assisted living, nursing homes, hospitals (ICU, trauma centers), and outpatient clinics
- Maintain compliance with HIPAA regulations while handling sensitive medical records and documentation
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
- Proven experience in diverse clinical settings such as long-term care, hospice, ICU, emergency medicine, or medical-surgical units is highly desirable
- Proficiency in EMR/EHR systems including Epic, Cerner, Athenahealth, or eClinicalWorks
- Strong knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques
- Experience administering injections, IV insertion/infusion, catheterization (including Foley and PICC lines), and wound suturing
- Ability to perform patient assessments including vital signs monitoring and physical examinations
- Knowledge of infection control practices and aseptic techniques in sterile processing environments
- Excellent communication skills for working with patients of all ages—including pediatrics (neonatal ventilator experience) and geriatrics—alongside families and interdisciplinary teams
- Ability to work independently in home settings while maintaining high standards of patient safety and confidentiality under HIPAA guidelines
- Current Basic Life Support (BLS) certification required; additional certifications such as ACLS or PALS are a plus
