? Position Summary

The Social Services Assistant supports the planning, development, and implementation of social service programs to ensure residents' emotional and social needs are met. This role works closely with the Social Services Director, medical staff, residents, and families to provide compassionate care and resources.

? Key Responsibilities

• Assist in planning and coordinating social service programs.
• Support residents and families with discharge planning, care plans, and assessments.
• Provide information and guidance on Medicare/Medicaid and other financial assistance programs.
• Maintain accurate records, progress notes, and social service data.
• Collaborate with community agencies and resources to meet resident needs.
• Offer emotional support to residents and families coping with illness, disability, or transitions.
• Participate in policy development and review for the department.
• Maintain positive working relationships with staff, residents, families, and external organizations.

? Qualifications

• High school diploma (required).
• Ability to read, write, and communicate effectively in English.
• Strong interpersonal skills with patience, tact, and a cheerful disposition.
• Knowledge of long-term care regulations and procedures preferred.
• Ability to make independent decisions and handle sensitive situations with professionalism.
• Must provide evidence of being free of tuberculosis infection upon hire.

? Physical & Work Environment

• Ability to move intermittently throughout the workday.
• Must be able to lift up to 25 lbs (occasionally up to 50 lbs).
• Work may involve exposure to infectious waste, diseases, and hazardous chemicals.
• Interacts with residents, families, staff, and outside agencies in a variety of settings.
