Community Support Professional

Overview

We are seeking a compassionate, reliable, and dedicated CORE Direct Service Provider to join our team and support individuals receiving Community Oriented Recovery and Empowerment (CORE) services. This role is ideal for someone passionate about person-centered care, community integration, and helping individuals build the skills needed to thrive independently.

As a CORE Direct Service Provider, you will work directly with participants in the community, their homes, and program settings to deliver individualized support services such as psychosocial rehabilitation, community psychiatric support, and skill-building impactful care in a supportive setting.

Key Responsibilities

• Provide person-centered CORE services in accordance with each participant's service plan.
• Support participants in developing daily living skills, socialization, community integration, employment and wellness goals.
• Assist individuals with navigation of community resources, appointments, and entitlements.
• Document all services delivered in compliance with program and Medicaid requirements.
• Collaborate with Care Managers, clinicians, and other service providers to ensure coordinated, high-quality care.
• Encourage independence, self-advocacy, and recovery-oriented progress.
• Maintain professional boundaries and uphold confidentiality at all times.

Qualifications

• High school diploma or GED required; Associate's or Bachelor's degree in Human Services, Social Work, Psychology, or related field preferred.
• Minimum of 1 year experience in the behavioral health field.
• Experience working with individuals with mental health or substance use challenges strongly preferred.
• Valid driver's license and ability to travel within Far Rockaway and surrounding areas.
• Strong communication, documentation, and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Compassion, patience, and a commitment to recovery-focused support.

Job Type: Part-time

Pay: $25.00 per hour
