Case Manager-(ESSHI) El Borinquen

Comunilife Inc is looking to fill a Case Manager position for its Bronx residential program. The Case Manager develops supportive relationships with clients; link them to  community- based medical and mental health care, and provide crisis intervention as needed. The Case Manager provides support with accessing primary medical care and other necessary services to increase clients' capacity for independent living and monitors progress, providing referrals as needed. The Case Manager completes required documentation in a timely manner and participates in case conferences, on-site and community-based meetings and trainings as required. This position is eligible to participate in Comunilife's  generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.

Tues.-Sat. 9am-5pm
