*Key Responsibilities:*
* Perform initial and ongoing patient assessments, including evaluation of physical abilities, home environment, and therapy needs.
* Develop and implement individualized care plans and treatment goals in collaboration with physicians, nurses, and other care team members.
* Provide therapeutic exercises, functional mobility training, balance, gait training, and pain management interventions.
* Educate patients and caregivers on home exercise programs, safety, and strategies to maximize independence.
* Document all patient care, progress notes, and updates in compliance with regulatory and agency standards.
* Monitor and adjust treatment plans based on patient progress and physician directives.
* Communicate effectively with patients, families, and interdisciplinary team members to ensure continuity of care.
* Adhere to all safety, infection control, and ethical standards in a home care setting.
*Requirements:*
* Current, active *Physical Therapist (PT) license* in the state of practice.
* Graduate of an *accredited Physical Therapy program (DPT or equivalent)*.
* Minimum of *1 year of clinical experience* (home health experience preferred, but not required).
* Current *CPR certification*.
* Valid *driver’s license*, reliable transportation, and proof of auto insurance.
* Strong interpersonal, communication, and organizational skills.
* Ability to work independently and manage time effectively in the field.
* Comfortable using EMR systems and mobile documentation tools.
*Preferred Qualifications:*
* Previous *home health care experience*.
* Knowledge of Medicare, Medicaid, and other payer documentation requirements.
* Bilingual skills (a plus, but not required).
*Work Environment:*
* Field-based, providing therapy in patients’ homes across assigned service areas.
* Flexible scheduling with PRN, part-time, and full-time opportunities.
