East Bay Community Action Program is hiring a full-time Case Manager This position is responsible for providing person-centered, unbiased care coordination and case management services to Medicaid, CNOM (Cost Share) and Adult Protective Services (APS) program participants. This role involves assessing client needs, developing care plans, coordinating community services, and ensuring compliance and comprehensive documentation. The Case Manager works closely with clients, families, and interdisciplinary teams to promote client well-being, maintain compliance with regulatory guidelines, and ensures that all services are delivered in a respectful, confidential, person - centered and culturally competent manner.

Key Responsibilities:

Assessment of Needs and Care Plan Development

  • Gather comprehensive participant history through direct interviews and input from family, providers, and other key sources.
  • Assess participant strengths, needs, resources, and goals.
  • Use person-centered planning tools to develop a comprehensive care plan that promotes participant health, welfare, and self-determined goals, while mitigating risks and accommodating preferences within service limits.

Care Coordination and Case Management (70%)

  • Coordinate care and services for program participants, that may include aging and disability population (EAD), and adults with intellectual and developmental disabilities (I/DD).
  • Conduct client visits and meetings in a variety of settings including clients' homes, assisted living facilities, group homes, etc.; and implement agency safety protocols to ensure the safety of oneself, colleagues, and clients.
  • Coordinate the provision of services with direct service providers such as Home Care agencies, Adult Day Health centers, and I/DD service providers.
  • Monitor clients' progress, ensuring ongoing evaluation and adjustments to care plans.
  • Conduct face-to-face meetings with participants or their representatives.
  • Become proficient in electronic case management systems and submit all required documentation in compliance with the State regulations.

Adult Protective Services Case Management

  • Address Adult Protective Services (APS) referrals, ensuring the safety and well-being of vulnerable clients.
  • Report abuse, neglect, or exploitation cases to appropriate authorities.
  • Maintain confidentiality and provide culturally competent, person-centered services.

Qualifications:

  • Associate required, Bachelor's degree in Human Services, Social Work, Psychology, Nursing, Gerontology, or a related field strongly preferred, equivalent combination of education and relevant experience will be considered.
  • Two years of experience in case management
  • Bilingual, Spanish and/or Portguese preferred.
  • Strong organizational and communication skills with the ability to manage shifting priorities and provide team guidance.
  • Valid driver's license, automobile insurance, and reliable transportation required.
  • Willingness to travel throughout the state of Rhode Island.
