We are seeking dedicated and skilled Physical Therapists to join our healthcare team in Cecil, Harford, Baltimore and Kent County, Maryland. The ideal candidate will play a vital role in providing comprehensive patient care within the home setting. This position offers an opportunity to work with patients within their homes, where they feel most comfortable. Our therapists work with clients to improve mobility, function, and safety within the home, resulting in an improved quality of life. A strong foundation in anatomy, physiology, medical terminology, and care planning is essential to deliver effective treatment plans tailored to individual needs.
The Physical Therapist will be responsible for evaluation, implementation, and progression of patient treatment plan in accordance with established practices and principles of physical therapy for patients in the living environment, i.e., home, assisted living facility or skilled nursing facility.
Duties
- All treatments are based on function and achieving improved independence in their living environment.
- Perform evaluations on patients to determine their level of neurologic, musculoskeletal and cardiac functioning while in their living environment.
- Develop and implement personalized care plans based on patient assessments and clinical findings.
- Provide patient care through manual therapy, therapeutic exercises, and other physiotherapy modalities.
- Document patient progress accurately
- Educate patients and caregivers on injury prevention, mobility techniques, and health maintenance strategies.
- Engage the patient so they understand the importance of their individualized program so they are actively achieving all goals.
Qualifications
- Valid MARYLAND state licensure as a Physical Therapist.
- Proven experience in inpatient, outpatient, hospital, or home health settings preferred; clinic experience is also valuable.
- Strong knowledge of anatomy, physiology, and medical terminology relevant to physical therapy practice..
- Experience with developing effective care plans
- Familiarity with documentation and patient record management.
- Ability to perform comprehensive assessments involving mobility, strength, and functional capacity.
- Excellent communication skills for patient education and interdisciplinary collaboration.

Pay: $ $72.00 per hour
