Novant Health is seeking an Ambulatory LPN. The Licensed Practical Nurse serves as a member of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with other team members to provide care that promotes optimal health. The Licensed Practical Nurse utilizes best scientific evidence and compassion to assist the care of the patient during their health journey. The Licensed Practical Nurse accepts accountability for provision of care, as part of the RN led team, in accordance with the current policies and procedures.

Essential Functions

  • Assessment: Participates in the collection of data as directed by the registered nurse. Participates in the determination of nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen. Sources of data collection include: subjective reporting, observations of appearance and/or behavior, measurements of physical structure and physiologic function, information regarding available resources. Able to determine a patient's need for immediate nursing intervention based upon data gathered regarding the patient's health status, ability to care for self, and current treatment regimen.

  • Planning: Participates in dialogue with the patient/chosen support system to identify what is important to them and incorporate his/her preferences and norms into the individualized care plan. Participates as part of the interdisciplinary care team in planning individualized care. Identifies relevant resources based on necessity and availability. Identifies nursing interventions and goals for the patient and reviews goals with the registered nurse. Collaborates with the registered nurse to consider ongoing modification of care plan based on patient's needs, capabilities and condition.

  • Collaboration: Participates in the teaching and counseling of patients and their support system as assigned by the registered nurse, physician or other healthcare team members. As delegated by Registered Nurse, the LPN collaborates with the healthcare team in participating in the planning and implementing in the patient's plan of care. Possesses the ability to prioritize job duties according to patient's needs and changing acuity levels. Administer and evaluate medication therapies, performs patient transfers and transport patients via wheelchair, hospital bed, or stretcher to other departments. Ability to lift and carry various equipment including portable cardiac monitors, bedside commodes, etc. Ability to provide routine nursing care such as start IV's, perform dressing changes, etc. to multiple patients in a timely manner. Ability to perform in critical situations, including administering CPR.

  • Evaluation: Participates in evaluation of nursing care and desired outcomes by collecting evaluative data from relevant sources; recognizing the effectiveness of nursing interventions; and proposing modifications to plan of care for review by the registered nurse.

  • Communication and Documentation: Communicates information to others responsible for, or involved in the care of the patient. The licensed practical nurse is accountable for directing communication to the appropriate person(s), communicating within a time period which is consistent with the patient's need for care, evaluating the nature of response to information reported, and determining whether further communication is needed. Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care. Documents appropriate information in a timely manner.

  • Professional Accountability/Evidence Based Practice: Demonstrates independence in the foundation of nursing care practice within the specialty. Practice according to your state's Nurse Practice Act, ANA Code of Ethics and ANA Bill of Rights. Seeks involvement in Shared Governance through council participation and/or by staying current with council activities. Identifies opportunities for process improvement, evidence-based practice and research. Demonstrates the utilization of evidence-based practice in nursing process. Engage in self-reflection and develop personal goals to enhance knowledge, obtain professional certification or pursue academic degree. Integrate gained knowledge into practice. Maintain a professional

portfolio.

  • Teamwork: Develops and maintains positive relationships with team members/internal customers. Consistently demonstrates a willingness to adjust activities to accommodate the needs of team members/internal customers. Resolves conflicts independently as appropriate. Accepts responsibility for actions and acts positively upon feedback from others.

  • Education: Graduate of accredited Practical Nursing Program. If your LPN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
  • Experience: One year of experience in a physician practice environment preferred.
  • Licensure/Certification/Registration: Current LPN license in appropriate state required.
