Summary
The Access Manager is responsible for facilitating an effective, efficient and safe admission/intake process . This position directly oversees the day-to-day operations of the BroMenn Campus out of town clinical liaison team. This team is responsible for admissions from most secondary and tertiary markets. The Access Manager also leads the inpatient admission assistant team. This team provides administrative support to all the BroMenn Access Department. The Access Manager leads regular communication and coordination of referrals and admissions with the team throughout the day. This position assists with difficult admission issues, prioritization of initiatives and territory management for the clinical liaisons working in the field.
Essential Job Responsibilities
Manages the effective, efficient and safe admission/intake process , quality improvement projects , compliance tracking and program evaluation.
Designs and implements centralized system access initiatives across the Mary Free Bed System where appropriate .
Responsible for bed management and intake process.
Assigns patients to physician/program services based on capacity and clinical needs.
Builds, tracks and analyzes data to support decision making of department.
Provides staffing and scheduling coverage for clinical liaison teams and inpatient administrative assistants.
Leads admission team huddles and provides census management coverage for Director of Inpatient Access when needed.
Coordinate efforts within the admissions office , physicians and interdisciplinary team to ensure timely and efficient handling of the referral/admission process.
Screen patients for appropriateness of inpatient admission to MFB IRF or SAR using the MFB admission criteria . Encourage MFB Continuum of Care for outpatient and home programs when appropriate .
Review referrals with Mary Free Bed Hospital physicians to determine appropriate level of post-acute care.
Use critical thinking skills to assist MFB physicians and liaisons with trouble shooting complicated referrals/admissions.
F acilitate admissions to Mary Free Bed, including required pre-admission documentation , setting appropriate expectations with patients/families and communication with staff at referring institution.
Processes information including financial and UR prior authorization, bringing unusual or sensitive information to key persons at MFB prior to decisions about admission.
Provide support and assistance to Network/External liaisons to ensure smooth communication with referring facility's staff, Mary Free Bed Hospital inpatient staff and patient/families.
Work closely with MFB access physicians to assign attending physicians appropriately.
Develop and maintain effective working relationships with MFB inpatient staff including physicians , midlevel's, care management, nursing and therapy departments .
Collaborate with MFB leadership to discuss admissions process and address concerns/issues that arise before and after admission to improve outcomes/processes/communication. Serve as 1 st level communication with physician and interdisciplinary team to promote efficient and effective messaging.
Engage with MFB Network Partners with the goal of problem solving and facilitating referrals to the appropriate level of rehabilitation.
Facilitate high quality continuum of care with all referral sources, ensuring that these sources are comfortable with making referrals to MFB. Develop protocols/processes with referral sources, partners and community resources to increase collaboration.
Maintain staff accountability for clinical and customer service standards.
Address noncompliance with policy, procedure and standards of practice and behavior with staff in timely , respectful and professional manner as needed. Address performance concerns with staff as needed.
Audit Pre-Admission Screen documentation and preadmission communication to interdisciplinary inpatient teams to ensure consistent and accurate information is shared in timely manner
Hire and train new staff .
Present key information and lead discussions in staff meetings as needed.
Promote the development and implementation of department policies. Interprets and observes established policies.
