⚕️ Position Overview: Licensed Practical Nurse (LPN)We are actively seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our interdisciplinary healthcare team. In this role, the LPN provides hands-on nursing care, supports rehabilitation goals, and works closely with RNs, physicians, and therapists to ensure residents receive safe, dignified, and high-quality care.
✅ Key Responsibilities of the LPN:- Provide direct patient-centered nursing care in a long-term care and rehabilitation setting
- Administer medications and treatments as prescribed by physicians
- Monitor residents’ conditions, including vital signs, and promptly report changes to the supervising RN or physician
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding when needed
- Collaborate with the interdisciplinary care team to support rehabilitation and recovery plans
- Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR)
- Provide emotional support and education to residents and their families
- Promote a safe, clean, and therapeutic environment by following infection control protocols
- Current and valid Licensed Practical Nurse (LPN) license

- Experience in long-term care, skilled nursing, or rehabilitation preferred
- Strong clinical skills, including medication administration and basic wound care
- Excellent communication, documentation, and teamwork skills
- Compassionate, patient-focused approach to care
- BLS/CPR certification required
