Responsibilities
- Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and behavioral health needs.
- Collaborate with medical teams to develop individualized discharge plans that incorporate community resources and post-acute care options.
- Provide mental health counseling and addiction support tailored to diverse patient populations, including pediatrics and behavioral health cases.
- Assist with intake procedures, ensuring compliance with HIPAA regulations and maintaining accurate medical records.
- Coordinate with hospital departments such as ICU, PICU (Pediatric Intensive Care Unit), emergency medicine, and trauma centers (Level I & II) to facilitate timely case management and utilization review.
- Support patients in navigating complex healthcare systems through case management, including outpatient clinics and managed care programs.
- Participate in diagnostic evaluations and inpatient or outpatient treatment planning to optimize patient outcomes.
- Engage in discharge planning activities, ensuring smooth transitions from hospital to home or post-acute facilities.
- Conduct intake assessments for behavioral health services and manage utilization management processes effectively.
- Maintain confidentiality of sensitive information in accordance with HIPAA standards while managing medical terminology and physiology knowledge essential for accurate patient assessment.
- Foster a compassionate environment by providing addiction counseling, mental health support, and behavioral health interventions as needed.
Experience
- Proven experience working within hospital settings, preferably in Level I or Level II trauma centers with acute care exposure.
- Demonstrated expertise in ICU, emergency medicine, or inpatient units such as PICU or post-acute care environments.
- Strong background in hospital medicine with familiarity in medical records management and diagnostic evaluation processes.
- Experience providing behavioral health services, mental health counseling, or addiction support across pediatric and adult populations.
- Knowledge of physiology related to trauma or critical illness enhances the ability to perform comprehensive patient assessments.
- Skilled in case management practices including discharge planning, outpatient coordination, and utilization review within inpatient or outpatient clinics.
